Robert, I just reread what you wrote and see that you mentioned that he had cancer. That means that he probably was at the hospital in Iowa City for care through the state (now University of Iowa Hospitals and Clinics) and probably died there. (I deleted some speculation here.) I would assume that he is buried somewhere in that cemetery at Iowa City. Have you contacted the Johnson County Genealogical Society to see whether anyone there is an expert regarding the cemetery? Perhaps someone there would have an idea what part of the cemetery he might be buried in. You could also contact the hospital regarding regulations for state patients who died in 1920 if you think he may have died in the hospital.
