I just called the 2 places and they sent forms for me to fill out. Clarinda was free and was prompt in reply,sent copy of admittance, with notations of evaluations. Mt Pleasant charged a small fee and took several weeks for reply and and did not send copy of admittance, just
verified relative had been there.
