To all whom it may concern:
This
is to Certify, That Thomas H. Casey, #1215006 Private, 39th
Company, 152d Depot Brigade, was discharged from the military
service of the United States by reason of disability, at Camp
Upton, New York, on the eleventh day of May, 1918, that at the
time of his discharge there was erroneously issued to him a
discharge certificate on Form 526, A.G.O., whereas he was
entitled to receive a discharge certificate Form 525, A.G.O.,
indicating that he had been Honorably Discharged the service.
Having surrendered said certificate so issued to him, this
certificate is issued in lieu thereof, and by virtue of this
certificate he is entitled to be regarded as having been
Honorably Discharged the military service as of the date and
place aforementioned.

Casey was born in Osborne, Clayton Co., in the State of Iowa.
When enlisted he was 22 years of age. By occupation a farmer. He
had grey eyes, dark brown hair, fair complexion. He was five feet
and 4 1/2 inches in height.
