The United States Army, as a Testimonial of Honest and Faithful Service, is hereby Honorably Discharged from the military service of the United States by reason of: For the convenience of the Gov. per form 16 S.O. 28 Hdq Camp Grant, Ill., Jan. 28, 1919.

Said Carl F. Meierkord was born in Waukon in the State of Iowa. When enlisted he was 24 10/12 years of age, and by occupation a Bookkeeper. He had Blue eyes, Light Brown hair, Fair complexion and was 5 feet 9 inches in height.
