This is to Certify, that: Floyd A. Leas, #4818229, Private, Co. "D" Development Battalion No. One
The United States Army, as a Testimonial of Honest and Faithful Service, is hereby Honorably Discharged from the military service of the United States by reason of: Physical deficiency per Surgeons certificate of Disability. S.C.D. and [illegible] 163rd Depot Brigade Nov 25, 1918 giving [illegible] as: asthma bronchial. Discharged by order of the Secretary of War paragraph 7 General Order War Department 1919.

Said Floyd A. Leas was born in Waukon in the State of Iowa. When enlisted he was 22 3/12 years of age, and by occupation a Truck Driver. He had Blue eyes, Brown hair, Ruddy complexion and was 5 feet 9 1/2 inches in height.
